1. Gather Materials

1. Download and electronically complete the Government Passport Application Form DS-82 and print one copy.
2. The Authorization letter, authorizes our service to expedite your passport processing. After you print the letter of authorization please check off box one and two on the form only. Please hand write Priority Passport Service where it reads courier company name. (click to download.) This form must be completed by hand.
3. Valid or Expired Passport (Must have been issued less than 15 years ago)
4. One (1) Passport Picture (click here for list of requirements)
5. Intent to travel (proof that you are traveling soon, click here for list of examples)
6. Government Fees/Payments: Check made payable to "US Department of State" - $190.00

2. Ship your Application using FedEx Priority

1. Call (407) 792-1000 to let us know that you are ready to ship your documents to us. A representative will review the documents and indicate where to send them.
2. Place the above listed documents in a FedEx envelope
3. Ship to:
